Environment of care
The finding, the owner and the closure in one chain
Environment of care rounding is not hard because the checks are hard. It is hard because the path from noticing something to proving it was fixed runs through a paper round, a photograph on a phone, a work order system that does not know about the round, and a person who has since left.
- Findings raise corrective work
- Evidence by unit and date
- Completes with no connectivity

The loop
From a round to closed evidence
- 1
Round the unit
One check at a time, with the room tag scanned so the location is established rather than asserted.
- 2
Something fails
The failed check raises a deficiency with the photograph, the room and the timestamp already attached.
- 3
Somebody owns it
Assigned with a due date, escalated if it passes, and visible in the same reporting as all other work.
- 4
It closes with proof
The closing evidence attaches to the same record as the finding, so the chain is complete.
Scope
What an environment of care programme covers
- Safety and security in patient care areas
- Hazardous materials and waste storage
- Fire safety equipment and means of egress
- Medical equipment condition and testing
- Utility systems and emergency power
- Emergency management readiness
- Ligature risk in behavioral health units
- Temperature, humidity and pressure relationships
- Eyewash stations and safety showers
- Corridor clutter and egress obstruction
- Interim life safety measures during construction
- Signage, lighting and wayfinding
Why the evidence is the hard part
Ask a hospital facilities director whether the rounds happen and the answer is yes. Ask them to produce the round for one specified unit on one specified date, with what was found and what happened next, and the answer takes a week.
That gap is a filing problem rather than a competence problem. The round was on paper, the photograph was on somebody’s phone, the correction was in a work order system that has no link to the round, and the person who closed it has left the organisation.
DFM closes the gap by making the record a by-product of performing the round. The check, the photograph, the room, the time, the finding, the owner and the closing evidence are one chain, so retrieving any date from it is a search.
Questions people ask
- Can different units be rounded with different checks?
- Yes. Each round is its own procedure, scoped to the buildings, floors or rooms it applies to, with its own frequency.
- How quickly can we produce the record for a specific unit and date?
- As quickly as you can search for them. The round, its answers, its evidence and its follow-up work are one record.
- Can we see open findings by department before a survey?
- Yes, and that is the more useful view. Open deficiencies by department and by age is what tells you where you are exposed.
